TIPS FOR PACU RESIDENT

EPIC ORDERS
ALL PACU ORDERS ARE DONE WITHIN ANESTHESIA NAVIGATOR- POST-OP
*helpful to set defaults and personalize your order sets

EPIDURALS
Order set: “IP PAI EPIDURA…”
Example of standard dosing: Bupivicaine 1/8% or 1/4% @ 8-10ml/hr
                                                Dilaudid 0.1 or 0.2 mg/hr
                                                Fentanyl PCEA 10 mcg q 10-15 min
*check with acute pain team service for any attending preferences on orders

PERIPHERAL NERVE BLOCKS
Order set: “IP PAI PERIPHERAL NERVE CATH/REGIONAL ANESTHESIA”
In order set, click nursing, monitor vitals and notify MD orders.  Leave diet, activity, PRN’s up to primary service)

*You will order the Local Anesthetic Infusion separately under a “new order”.  Ignore the On-Q order options in the order set.

Examples of standard dosing:
                        INTERSCALENE- ¼% Bupivicaine @ 6cc/hr
                        SUPRACLAVICULAR-  ¼% Bupivicaine @ 6cc/hr
                        FEMORAL- ¼% Bupivicaine @ 6-8 cc/hr

*You must also order PCA for all patients with a peripheral catheter placed by the regional team. 

PCA
Order set: “IP PAI PATIENT CONTROLLED ANALGESIA (PCA)
Example of standard dosing: Dilaudid PCA 0.2-0.4 mg q 10-15 minutes

*if patient does not have a nerve catheter in place, you do not put in the PCA orders, the primary service should do it
           
OTHER RESPONSIBILITES:
-UPDATE PAIN TEAM LIST WITH POST OP PATIENTS WITH EPIDURALS, CATHETERS, SPINAL BLOCKS
-ENTER ORDERS FOR ALL CATHETERS WITH ASSOCIATED PCA’s
- ADD PATIENTS TO THE “CON PAIN” LIST ON EPIC WHICH CAN BE FOUND UNDER “PATIENT LISTS”à “FOLLOW UP CONSULTS”à “CON PAIN”
             TO ADD PATIENT TO LIST, RIGHT CLICK PATIENT NAME
            CLICK TREATMENT TEAM

ENTER “CON PAIN” for TREATMENT PROVIDER AND “CONSULTING SERVICE” UNDER RELATIONSHIP.
